1. Contract- complete but do not sign this document.  The child's name must appear on the contract exactly as it appears on his/her birth certificate.

4. Physical Form - this is required on the first day of practice.   Players are not allowed to practice without the physical examination form signed, dated and stamped by a physician.    Please note that a signature alone is not sufficient.   This form will be collected on the first day of practice.

6. We require an original birth certificate for all first time players.  This should be provided in August and we will hold the certificate until the team has been certified and then it will be returned.

7. First time cheerleaders should provide a copy of their birth certificate by the first day of practice. Birth certificates are returned after the teams have been certified.

The above forms will be available for pick-up and completion at the park on the days of sign-ups.
