Below are the forms you need to register your child for football or cheerleading. You can download and complete forms here, or you can pick up the forms at Northridge Park. If you download the forms, just remember, do not sign them - they must be signed in the presence of a board member at the park.
To signup we require a deposit on the registration fees: For cheer and mascots the deposit is $186.00 with a balance for the uniform (payable separately to the vendor) of $180 (uniform fee includes shoes) and $109 respectively (mascots provide their own solid white tennis shoes).. For football, the deposit is $150.00 with a balance of $125.00. All registration fees are due by August 11, 2008.
