2010 SIGN UP INFORMATION
PARTICIPANT'S AGE AS OF AUGUST 1ST
Football: 6-14 years (based on age & weight)
Cheerleaders: 6-14 years (based on age)
Cheer Mascot: 2-6 years
To register, a deposit of $150 is required for football players and for cheerleaders and mascots the registration fee is $186. The balance of all fees is due NO LATER THAN AUGUST 13, 2010. Late sign-ups - after August 1, 2010 - the full registration fee is required IN CASH - no checks.

2010 Sign-Up Dates
All sign-ups will be at NORTHRIDGE PARK Lemarsh Street - Upper Field
Saturday, April 24 10 am - 2 pm Sunday, April 25 12 noon - 4 pm Saturday, May 22 10 am - 2 pm Sunday, May 23 12 noon - 4 pm Saturday, June 26 10 am - 2 pm Sunday, June 27 12 noon - 4 pm Saturday, July 24 10 am - 2 pm We have added another day in July Sunday, July 25 ______12 noon - 4 pm
The first day of practice is devoted to those already registered and ready to practice - we will be collecting all physical forms and ORIGINAL birth certificates for football players who have never played before, and photocopies for cheerleaders. It is for that reason that we do not sign up anyone on the first day of practice (August 2), nor do we accept balance of fees on that day.

Participation Fees
We are pleased to announce that our 2010 fees will remain the same and are as follows:-
Cheer and Mascot Uniforms are paid separately, directly to the vendor.
Participation fees include registration, football uniforms, raffle tickets, team fees, cheer accessories, and homecoming fees.
Payment Plans: If you want to be put on a payment plan, you must sign your child/children up at the first 3 signups (April, May & June) so we can work a payment plan that will be complete by August 13. All signups in July & August must be paid in full at time of signup as our closing date for registration payments is August 13.

Your child will not be allowed on the field to practice until the physical form has been completed and signed by your physician and handed in at the the registration desk.
To help you with the sign-up procedure for 2010, you can download and complete all the forms before coming to Northridge Park. Don't forget that the Conference requires all players, cheerleaders and mascots to sign his/her name on the contract exactly as it appears on his/her birth certificate and it must be signed in front of a Registration Committee Member at the Park.
Football Players are required to complete or provide the following forms:
First time player will need to turn in an original birth certificate by the day of practice - birth certificates are returned after the player has been certified. PDF

The above forms will still be available for pick-up and completion at the park on the days of sign-ups.
Note: It is mandatory for all participants - football and cheer - to have a current physical exam prior to the start of practice in August. The Physical Examination form must be signed, dated, and stamped by a MD Physician - Registered Nurses or Nurse Practitioners MAY NOT perform the physical or sign the medical examination report form.
